LIABILITY WAIVER & MEDIA RELEASE

Release of Liability

 I / my child, [Full Name], hereby give my full consent and approval for myself or my child to fully participate in classes at Synergy at Play.

I understand that participation in these classes involves certain risks, which I voluntarily accept for myself and my child.

On behalf of myself / my child, I agree to hold harmless and fully indemnify Synergy at Play, from any liability arising from my / my child’s injury or injury to any third party resulting from my child’s actions or participation in the activities.

Health & Safety Policy

To protect all participants and prevent the spread of illness, children may not attend Synergy at Play if they exhibit any of the following symptoms, as per BC Health Regulations:

  • Cold symptoms: Runny nose, coughing, sore throat, watery eyes.

  • Respiratory issues: Difficulty breathing, wheezing, persistent cough.

  • Fever: 100°F (38.3°C) or higher. Children must be fever-free for 24 hours without medication before returning.

  • Infected skin/eyes: Undiagnosed rash, eye infections.

  • Gastrointestinal issues: Diarrhea, vomiting, nausea, stomach cramps. Children must be symptom-free for 24 hours without medication before returning.

  • Communicable illnesses: Any other communicable illnesses or conditions. A doctor’s note may be required for return.

Scent Policy

Please refrain from using scented detergents, body and hair products as these create unsafe environment for people that are sensitive to chemical toxicities.

General Etiquette & Behaviour

Please be respectful at all times—this is a shared space.
Children learn from the adults around them, so we encourage all caregivers to model kindness, patience, and positive behaviour. Foul language, aggressive behaviour, or physical altercations toward children, parents, or staff will not be tolerated.
We acknowledge that parenting styles and beliefs vary. We ask all families to practice tolerance, compassion, and mutual respect when interacting with others.

Media Release

I hereby irrevocably authorize Synergy At Play to edit, alter, copy, exhibit, publish or distribute media for purposes of publicizing Synergy At Play’s programs or any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my / my child’s likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of any media. 

I hereby hold harmless and release and forever discharge Synergy At Play from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
